In practice
What this template helps you manage
- Keep requests in one structured register: Record the request ID, date received, internal student record key, requestor role, request type, requested field, summary, supporting-information status, assignee, priority, workflow status, milestone dates, and resolution note in one row.
- Identify urgent and overdue work: The selected priority supplies a target number of calendar days. Formulas calculate the target completion date and classify open requests as On Track, Due Soon, Overdue, or Missing Due Date.
- Measure processing performance: Track completed requests, closed-on-time results, on-time completion rate, average days to close, individual days to close, and the current age of open requests.
- Standardize routine data entry: Drop-down lists support consistent entry for requestor role, request type, requested field, supporting information received, assigned staff member, priority, and status.

About this template
The Requests sheet has 200 prepared data rows, from row 5 through row 204. This total includes populated sample requests and blank operational rows. Replace or remove the samples before using the workbook for live tracking.
The calculated fields are Target Days, Target Completion Date, Due Alert, Timeliness, Days to Close, Open Age, and Review Check. Formulas are filled through all 200 prepared rows, and users should not overwrite these columns.
The selected priority is matched to its Target Calendar Days value on the Lists sheet. The included settings are 2 days for Urgent, 5 for High, 10 for Normal, and 15 for Low. The workbook adds that number of calendar days to Date Received, then classifies an open request as On Track, Due Soon, Overdue, or Missing Due Date.
The Dashboard shows total requests, open requests, overdue open requests, requests due in the next three days, closed requests, closed-on-time requests, on-time completion rate, and average days to close. It also includes summaries by status and request type, open counts by priority, open-request age bands, and two charts.
Yes. The Lists sheet contains the choices for requestor roles, request types, requested fields, statuses, assigned staff, priorities, and Yes or No responses. It also stores target calendar days, which accept whole numbers from 1 through 365. Edit these settings carefully because renaming a list item does not automatically update existing request rows.
Do not enter student names, birth dates, government identifiers, grades, medical information, exact before-and-after record values, sensitive document contents, or supporting documents. Use an internal Student Record Key and keep sensitive details and files in approved institutional systems.
