In practice
What this template helps you manage
- Keep requests in one structured log: Maintain one row per request with its course, section, term, request type, dates, reason, submitter, approval information, status, assignee, and resolution note.
- Connect requests to applicable deadlines: The workbook matches each selected Term and Request Type to the deadline table in Settings, then calculates submission timing and days to deadline.
- Prioritize open work: Calculated fields identify whether a request is open or closed, measure request age, recommend a next action, and surface deadline or processing alerts.
- Monitor operations from a dashboard: Review request volume, open work, approaching and missed deadlines, approval queues, completed requests, on-time submission rate, average days open, and records needing data review.

About this template
The Request Log contains 150 prepared operational rows, from row 5 through row 154. Each row includes formulas in columns Q through Z.
The workbook also contains fictional sample records that demonstrate the intended structure. These samples can be replaced with operational entries.
Users enter request details in columns A through P, including request references, dates, term, course information, request type, approvals, status, assignment, completion date, and resolution note.
Columns Q through Z calculate Applicable Deadline, Submission Timing, Days to Deadline, Request Age, Open or Closed, Next Action, Alert, Required Fields Check, Duplicate Check, and Record Review. The Instructions sheet identifies input cells as light gold and calculated cells as light gray.
The workbook matches the request's Term and Request Type against the tblDeadlines table on the Settings sheet. When a matching rule exists, it returns the corresponding Applicable Deadline.
If no matching rule exists, the deadline remains blank and the request can receive a Deadline Missing alert. The Settings sheet also checks for missing deadline dates, duplicate rules, and incomplete coverage of the nine included term-and-request-type combinations.
The Dashboard reports total requests, open requests, past-deadline open requests, requests due within three days, requests awaiting approval, approved requests not completed, completed requests, on-time submission rate, average days open, and records needing data review.
It also summarizes Add, Drop, and Withdrawal totals, status counts, and alert counts. Two charts display total requests by request type and request counts by status.
Validation rules support Request ID and Learner Reference formats, date ranges, text lengths, the three included terms, request types, reason categories, submitter types, approval values, statuses, and the included fictional staff list.
Calculated review fields can identify missing required information, duplicate Request IDs, missing deadlines, approval mismatches, closed requests without a Completed Date, and other date or record conditions requiring attention.
The included request records and staff names are fictional samples. The Instructions sheet directs users to use a non-identifying Learner Reference instead of a learner name.
It also advises against entering birth dates, government identifiers, financial account information, medical details, or lengthy personal narratives. The workbook is an operational aid and does not replace institutional policy, official records, or required review processes.
