This free Excel template helps teams manage student record change request tracker. This free student record change request tracker Excel template helps registrar, student services, and course operations teams coordinate record change requests from intake through completion. Use the structured register to document each request with an internal student record key, assign responsibility, set priority, update status, and record milestone dates.

Built-in formulas calculate target completion dates, due alerts, timeliness, days to close, open age, and review checks. The workbook also includes an automatically updating dashboard, editable validation lists, instructions, populated sample records, and blank prepared rows for operational use.

In practice

What this template helps you manage

  • Keep requests in one structured register: Record the request ID, date received, internal student record key, requestor role, request type, requested field, summary, supporting-information status, assignee, priority, workflow status, milestone dates, and resolution note in one row.
  • Identify urgent and overdue work: The selected priority supplies a target number of calendar days. Formulas calculate the target completion date and classify open requests as On Track, Due Soon, Overdue, or Missing Due Date.
  • Measure processing performance: Track completed requests, closed-on-time results, on-time completion rate, average days to close, individual days to close, and the current age of open requests.
  • Standardize routine data entry: Drop-down lists support consistent entry for requestor role, request type, requested field, supporting information received, assigned staff member, priority, and status.

How to use it

How to use the workbook

Start on the Instructions sheet, then replace or remove the populated sample requests before live use. On the Requests sheet, enter each new request on the next blank row and assign a unique Request ID beginning with RCR-. Use an internal Student Record Key instead of a student name, complete the user-entry fields, and select standardized values from the drop-down lists. Assign a priority and owner; Target Days and Target Completion Date will calculate automatically. As work progresses, update Status, Assigned To, Last Updated, Decision Date, Completion Date, and Resolution Note when applicable. Review Due Alert and Review Check before operational meetings, and use the Dashboard to monitor volume, deadlines, aging, request mix, and completion performance. Do not overwrite the calculated columns from Target Days through Review Check. Target dates use calendar days, and calculations that use TODAY() change with the current date.

Inside the file

Included features and controls

Four purpose-built worksheets: The workbook contains Dashboard, Requests, Lists, and Instructions sheets.
200 prepared request rows: The Requests sheet provides rows 5 through 204 for up to 200 records. The prepared area includes populated sample records as well as blank operational rows with formulas filled down.
Excel request table: The request register is formatted as RequestTable across A4:V204.
Fifteen user-entry fields: Users can enter Request ID, Date Received, Student Record Key, Requestor Role, Request Type, Requested Field, Change Summary, Supporting Information Received, Assigned To, Priority, Status, Decision Date, Completion Date, Last Updated, and Resolution Note.

Track Student Record Change Requests From Intake to Completion

The Requests sheet gives operations teams a consistent place to log and maintain record change requests. Each row can capture an RCR-prefixed request ID, intake date, internal student record key, requestor role, request category, requested field, concise change summary, supporting-information response, assignee, priority, status, relevant dates, and resolution note. Sample records demonstrate how the register is used, while blank prepared rows support ongoing entry.

Monitor Due Dates, Open Age, and Timeliness Automatically

Priority drives the Target Days lookup, and the tracker adds those calendar days to Date Received to calculate Target Completion Date. Open records can display On Track, Due Soon, Overdue, or Missing Due Date. Completed records can display On Time, Late, or Needs Review. Separate calculations provide Days to Close for completed work and Open Age for requests that are not Completed or Canceled.

Review Workload With the Student Request Dashboard

The Dashboard updates from records entered in the prepared Requests range. Headline metrics cover total workload, open workload, overdue requests, requests due within three days, completed requests, on-time completions, on-time completion rate, and average days to close. Supporting summaries organize volume by status and request type, show open counts by priority, and group active requests into five age bands. Two charts visualize counts by status and request type.

Use Validation and Review Checks for More Consistent Records

Drop-down lists standardize common operational fields, while date and text rules constrain selected entries. The Review Check formula flags issues such as duplicate request IDs and missing required information, and conditional formatting marks rows requiring attention. Formula cells are visually distinguished from user-entry cells and should not be overwritten. List values and priority targets can be edited carefully on the Lists sheet, but those changes do not retroactively rename values already stored in request rows.
